Repeat script request

To request a repeat prescription, fill out the form below. All fields must be filled out, or your request will not be submitted.

 

You can either collect your prescription from reception or ask for it to be sent directly to a pharmacy.  Make sure you advise in your request which pharmacy you want the prescription sent to.

 

Prescriptions take 2-3 working days to process and cost $20. Prescriptions are not processed on public holidays, Saturdays or Sundays.

 

If you require an urgent script, please note this in your request. These are processed on a same-day basis if ordered before 2pm or, if ordered after 2pm, will be ready the next working day. A $30 fee applies to all urgent requests.

 

For children aged 0-13 years there is no charge for repeat scripts.

 

If you want us to send your script to a pharmacy outside Te Awamutu, please make sure to provide the full name and location of the pharmacy.

 

Payments can be made online to our bank account - Westpac 03 0442-0182067-001. Please put your name as the reference with your payment.

 

 

Ordering repeats for long term medications is easy if you have a log in for the Manage My Health patient portal. Ask at reception if you would like to sign up for Manage My Health.
